U.S. Supplier Registration

Follow the step by step instructions to register as a SLAC Supplier.

 

1.  Please have your organization's current, completed and signed IRS W9 Form. 

Note: Old versions will not be accepted, the lastest version can be found on the IRS site:  https://www.irs.gov/pub/irs-pdf/fw9.pdf .
​W-9 documentation is REQUIRED for approval, failure to attach W-9 will result in a delay and possible denial of registration.

2.  Start your registration by selecting  SLAC Self-Registration.  

Note: Internet Explorer is the recommeded browser for the registration process.

3.  Select REGISTER AS A SUPPLIER

 

4.  Ensure the BUSINESS radial button is selected.

Note: Individuals or Sole Proprietorship must register using the Business button.  The Individual radio button has been disabled. 

5.  Select NEXT.

 

 

 

6.    On the Identifying Information Page, enter Business Information. 

Note: Provide Legal Business Name (see block 1 of IRS W9 FORM) and Tax ID Number (TIN) (see Part 1 of IRS W9 FORM).

7.    Use the Add Attachment hyperlink to the right of Business Name to attach completed current IRS W-9 Form. 

 

 

8. Upload IRS W9 Tax Form.

If you have trouble attaching your W9, please email it to suppliers@slac.stanford.edu. 

 

 

 

9.  Complete the Identifying Information Page by entering a North American Industry Classification (NAICS) Code and if applicable select small business attributes.

10.  Select NEXT to continue to the next page.

 

 

 

11.  On the Address Page, enter your primary ordering address.  

Enter your zip code with the +4 Suffix, which can be found at United State Postal Service website.  

Ensure you enter a Remit to Address if it is different than your Ordering/Primary Address.

12.  Select NEXT to continue.

 

 

13.  Add Contacts that will be used for Ordering, Payments, and have the ability to request updates. 

 

14.  Each contact will have a username and password.  

Note: Use at least one upper case, one lower case, and one number in creating your Username and Password.

15.  Select OK to return to the Contacts page, select NEXT to continue. 

 

 

 

 

16. Enter Payment Information.  In order to ensure timely payment, enter the information highlighted in the blocks below.

 

 

 

17. Before completing the Payment Information Page, enter your DUNS Number.  

 

 

 

18. Finally, enter an email address for registration updates and Accept the Terms of Agreement for using the Self Registration and then Select SUBMIT.

 

Your Registration will be reviewed by SLAC Supply Chain Management and Accounts Payable before it is active.  
If you need assistance, please contact Suppliers@slac.stanford.edu with  your Registration Number, which is emailed to the address used for registration updates.
